An unlock request is waiting for a reply from a master node. That
master node fails causing a recovery event. As of yesterday we simply
treat these unlock requests as completed and dont rebuild them during
recovery. Before queueing the completion ast for these unlock requests
the lkb must be removed from the rsb queue and the lockqueue. An
assertion failure was caused by not doing this.
