Changes to how dlm flags are used based on dlm changes.
Change the way we deal with gfss "hold_lvb" requirement: use a
separate NL lock to represent a "held lvb" instead of replacing
unlocks with conversions to NL. This way unlocks from gfs are
always handled consistently, with a corresponding dlm unlock.
